Checklist for investment trust applications

Instructions for completion of application forms

NOTE:

  • Page numbers in the check list refer to pages in the Product Disclosure Statement (PDS).
  • A copy of this check list can also be found at the back of the PDS

To invest, complete the application form and post it to us.

  • If you are investing by cheque, post the application form with a cheque made out to Australian Ethical Applications on Trust. Cheques must be in Australian dollars.
  • If you are investing through a regular savings plan, post the application form and a completed direct debit request form.
  • If you are investing through BPAY, post the application form.

Forms are at the end of the Product Disclosure Statement. Use only an original application form. Forms printed from a downloaded Product Disclosure Statement are acceptable.

Please ensure you have read the declaration on page 51 of the PDS before signing the forms.


Who should sign the application form?

Application forms must be signed by you (the investor) unless investing under a power of attorney (see below). Joint applicants should provide signatures for each person nominated on the application form. Read the declarations accompanying this form on page 51 of the PDS before signing the form.

Powers of attorney
Generally we will accept applications made under a power of attorney. A certified copy of the power of attorney must be submitted with the application for processing. Please contact us for further details and be aware that the laws applying to powers of attorney may differ between the states.

Application Checklist

1. Investor details
□ Place your investor number in the box provided if you are already an investor with Australian Ethical.
□ Indicate what type of investor you are.
□ Fill in your investor details.

    Tax file number or exemption
□  It is optional to provide your tax file number but if you choose not to we must withhold tax from your distributions at the highest marginal rate. If you wish to claim an exemption please state the exemption in the tax file number box. If the organisation submitting this application is not required to lodge a tax return please give the reason.

2. How to fund your investment
□ Select the method you wish to use to fund your investment – cheque, BPAY or regular savings plan.
□ If you are starting a regular savings plan, complete the direct debit request form (Form C) in addition to the application form.

3.  Investment instructions
□ Under ‘Investment instructions’ enter the amount you are investing in the box beside the trust(s) you wish to invest in.
□ Tick one option for each trust for payment of your distribution. If you do not select an option your distributions will be reinvested.

4. Distribution direct deposit
□ Enter your bank/ credit union account details in this section. Please note that this section should not be used for regular saver direct debits.

5 How did you hear about us?
□  Please indicate how you first heard about Australian Ethical. This is for internal company use only.

6. Signature(s)
□  Refer to Who should sign the application form? above. Read the declaration on page 51 of the PDS before signing the form.

7. Adviser information
□ If you have agreed with your adviser to pay them a trailing commission (ongoing service fee) on your entire investment in the trusts, put in the percentage you want to pay. Your adviser should complete and sign this section in your presence.

8. Post the completed application form
□  Post the completed application form and your cheque or the direct debit request form if applicable to:

Australian Ethical Investment Ltd reserves the right to refuse any application. If the application form is incomplete, we will retain the money while we attempt to get the required information. If we are unsuccessful, the money will be returned.
